Effective Date: January 1, 2024 - December 31, 2024
LIABILITY RELEASE: In consideration of Livingston First Assembly of God in Livingston, TX, allowing the minor Participant to participate in youth ministry Activities, Events, Retreats, and Trips, I, the undersigned, do hereby release, forever discharge and agree to hold harmless Livingston Assembly of God Church, its pastors, administrators, directors, agents, officers, members, volunteers, employees, other participants, any sponsors, advertisers (collectively herein the “LFA CHURCH”), and if applicable, owner and lessors of premises on which the Activity takes place (each one considered one of the “RELEASEES” herein) from any and all liability, claims and demands for accidental personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the Participant while involved in the activities. I the parent or legal guardian of this minor Participant hereby grant permission for the minor Participant to participate fully in youth ministry activities which occur both on and off the church premises. Furthermore, I, on behalf of my minor Participant, hereby assume all risk of accidental personal injury, sickness, death, damage and expense as a result of participation in recreation and work activities involved therein. The undersigned further hereby agrees to hold harmless and indemnify said LFA CHURCH for any liability sustained by said LFA CHURCH as the result of the negligent, willful or intentional acts of said minor Participant, including expenses incurred attendant thereto.
MEDICAL RELEASE: In the event of an emergency, effort will be made to contact the parent/guardian and/or the emergency contact with the information provided on this form. In the event that contact cannot be made, LFA Church staff and/or adult volunteer(s) in whose care your child has been entrusted will assume responsibility to transport your child to a medical facility if deemed necessary. In this same circumstance(s) mentioned above, an adult, in whose care your child has been entrusted to, may select to secure proper treatment for, to hospitalize, and to order any emergency x-ray examination, anesthetic, medical, surgical or dental diagnosis or treatment and hospital care, to be rendered to your child under the general or special supervision and on the advice of any licensed physician or dentist. The parent/guardian will be liable and responsible to pay all costs and expenses incurred in connection with such medical and dental services rendered to the aforementioned child.
PHOTO RELEASE: I, on behalf of my minor Participant, understand that there may be photographs, video footage, and other recordings taken of the minor Participant and all other event attendees during church related activities, and that this photography, video footage, and other recordings could be used for promotional purposes including being posted on the internet. I grant LFA Church permission to use and re-use, publish and re-publish, and to modify and alter any photographs, video footage, and other recordings from church related events. I also waive my right to inspect or approve any editorial text or copy that is used in connection with the photographs, video footage, and other recordings and hereby release and discharge LFA Church from any and all claims arising out of the use of the photographs, video footage, and other recordings for the purposes described above.
TRANSPORTATION PERMISSION: I, on behalf of my minor Participant, give permission for my minor Participant to ride in any vehicle driven by an approved and licensed adult chaperone while attending and participating in activities sponsored by LFA Church.
EARLY HOME POLICY: Should it be necessary for my minor Participant to return home due to medical reasons, disciplinary action or otherwise, the undersigned parent/guardian shall assume all transportation costs and responsibility.
COVID-19: I, on behalf of my minor Participant, acknowledge COVID-19 as a contagious virus and most prone to exposure and/or infection through person to person physical contact, close proximity breathing, and contact with stable surfaces. I voluntarily and knowingly assume the risk of exposure to or infection by COVID-19 by allowing my minor Participant to attend in person meetings and events. I acknowledge that exposure to or infection by COVID-19 may result in personal injury, illness, permanent disability, or death. I, the undersigned, do hereby release, forever discharge and agree to hold harmless LFA Church, its pastors, administrators, directors, agents, officers, members, volunteers, employees, other participants, any sponsors, advertisers (collectively herein the “CHURCH”), and if applicable, owner and lessors of premises on which the Activity takes place (each one considered one of the “RELEASEES” herein) from any and all liability in the event of exposure to or infection by COVID-19.
I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND HAVE SIGNED IT FREELY AND WITHOUT INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT. AND I, THE MINOR’S PARENT AND/OR LEGAL GUARDIAN, UNDERSTAND THE NATURE OF ACTIVITIES AND THE MINOR’S EXPERIENCE AND CAPABILITIES AND BELIEVE THE MINOR TO BE QUALIFIED, IN GOOD HEALTH, AND IN PROPER PHYSICAL CONDITION TO PARTICIPATE IN SUCH ACTIVITY. I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS EACH OF THE RELEASEE’S FROM ALL LIABILITY CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON THE MINOR’S ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATION AND FURTHER AGREE THAT IF, DESPITE THIS RELEASE, I, THE MINOR, OR ANYONE ON THE MINOR’S BEHALF MAKES A CLAIM AGAINST ANY OF THE RELEASEES NAMED ABOVE, I WILL INDEMNITY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS LIABILITY, DAMAGE, OR COST ANY MAY INCUR AS THE RESULT OF ANY SUCH CLAIM.
